Symptomatic Oculomotor Nerve Cyst in a 3-Year-Old Child: Case Report With Emphasis on Surgical Management

Abstract BACKGROUND AND IMPORTANCE Third nerve palsies in the pediatric population are most commonly caused by trauma, tumors, or vascular abnormalities. Cystic oculomotor neuropathies, however, rare. We report case of a symptomatic cyst along and within nerve, which has not been described previously. CLINICAL PRESENTATION Here, we 3-yr-old girl presenting with progressive painless palsy. A magnetic resonance imaging revealed cystic formation cisternal cavernous course nerve. Due to lack alternative treatment options, surgery was offered. Intraoperative direct stimulation allowed for identification non-functional part wall open fenestration biopsy were executed. Histopathology neuritis. Serology negative various pathogens. The palsy rapidly resolved. At follow-up 5 yr after surgery, is asymptomatic remains collapsed. CONCLUSION This first treated effectively decompression highlighting importance intraoperative neuromonitoring cranial surgery. Uncertainty regarding etiology this disease.
